 COVID-19 CLIENT SCREENING

In the interest of the safety of our staff and other clients, we need to ask you a few questions as part of our COVID-19 safety plan

CLIENT SCREENING QUESTIONS

  1. Have you or anyone in your home been in direct contact with someone who has been overseas in the past 14 days?
  1. Have you or anyone in your home been in contact with anyone who is a confirmed/suspected case of COVID-19?
  1. Are you or anyone in your home currently experiencing, or have you or anyone in your home, in the last 14 days, experienced any cold or flu-like symptoms (including fever, sore throat, fatigue, cough and/or shortness of breath)?

  1. Have you or anyone in your home currently been advised to self-isolate or awaiting a COVID test result?

  1. Would you have someone else present at home at the time of the appointment? If YES, ask Q. 1-4 for the person who will be present at the time of the appointment.

Whilst at your place we will observe good personal hygiene including but not limited to washing hands and using hand sanitiser frequently.

We will also wear appropriate PPE and request you and anyone in your home to use face masks at all times when our therapist is present.

**IF THE CLIENT ANSWERED 'YES' TO ANY OF THE QUESTIONS (Q. 1-4), PLEASE DO NOT ATTEND TO THE CLIENT AND CONTACT YOUR MANAGER IMMEDIATELY. **
